Lung cancer - Chronic

Definition  

A heterogeneous group of epithelial carcinomas that originate from either the bronchial surface or bronchial mucus glands


 

Medical History  

* Smoking

* Exposed to tobacco smoke

* Occupational Exposure

* Environmental Exposure

* Family history of Lung cancer


 

Findings  

* Cough

* Weight loss

* Dyspnea - Chronic

* Chest pain

* Hemoptysis

* Bone pain

* Lymphadenopathy

* Fatigue

* Loss of appetite

* Wheezing

* Dysphagia - Chronic

* Hoarse

* Headache


 

Tests  


 

Suspected lung cancer  

* Plain chest X-ray: The chest x-ray often contains the first clue to lung carcinoma in asymptomatic patients .

* CT of chest

* Cytologic test, Sputum specimen: Sputum cytology may be most useful in patients who present with centrally located tumors or hemoptysis; further testing is recommended for nondiagnostic results .

* Transthoracic needle biopsy: Transthoracic needle aspiration (TTNA) is recommended in patients with a small peripheral lesion who require tissue diagnosis before further management; further testing is recommended for nondiagnostic results .

* Bronchoscopy: Bronchoscopic techniques are most useful in diagnosing centrally located tumors; further testing is recommended for nondiagnostic results .

* Fine needle biopsy

* Thoracentesis


 

Noninvasive staging of lung cancer  

* CT of chest: A CT scan of the chest with contrast that includes the upper abdomen (liver and adrenal glands) is recommended to evaluate mediastinal metastases in patients with known or suspected lung cancer who are eligible for treatment

* Positron emission tomography: Positron emission tomography (PET) scanning is the most accurate noninvasive technique to evaluate mediastinal lymph nodes in lung cancer patients .


 

Invasive staging of lung cancer  

* Mediastinoscopy: Mediastinoscopy is generally the method of choice for invasive mediastinal staging of lung cancer .

* Endoscopic ultrasound of upper gastrointestinal tract: Endoscopic ultrasound with fine needle aspiration is particularly useful for sampling inferior pulmonary ligament, esophageal, subcarinal, and aortopulmonary window nodes and may prevent the need for surgical staging .

* Thoracoscopy

* Bronchoscopy with transbronchial lung biopsy
